Once we have received your referral and hopefully a social history report we will contact you for a chat on the telephone.

Assessment Procedure for 24 Hour Supported Accommodation and our Step Down Facility

We would make an appointment to come and do an assessment on your client. This would happen very quickly. The person doing the assessment would be one of our senior managers.

It may be that before that happens you would like to come visit us and see the service we provide to see if it would be suitable for your client.

They may have family, friends or advocates who would also like to visit and get involved in the assessment procedure.

Once the assessment is complete, using the Recovery Star Model which is person centred. We would then be able to give you a core cost price and a price for any additional one to one personal care and support if needed.

We would then ask if your client could visit us for lunch and to have a good look around the home to see if they would be happy with living at Woodlea it also gives our other tenants the opportunity to speak to your client and get acquainted.

It may be that you want some overnight stays to be arranged and this we can be arranged, we would give you the charges for this service at the time if that was the way your wanted to proceed.

If all parties were happy with the accommodation, we would ask that panel funding is arranged with our finance department and then an appropriate moving in date could be arranged. We would look at a first review after four weeks.
